Acute lithium intoxications are infrequent, especially in children, but potentially severe. Maintenance therapy with lithium (acute-on-chronic intoxication) or the absence of it (acute intoxication in naïve patients) is one of the most important factors when characterizing them. The mechanism is also important, being the substantial voluntary ingestions the most dangerous ones. Due to the severe toxicity, fundamentally neurological, and the toxicokinetic features of lithium (as the null adsorption by activated charcoal, free distribution, and tubular reabsorption), exceptional techniques in paediatric intoxications such as total intestinal lavage, hyperhydration and haemodialysis may be necessary.

We present three cases recently observed in the emergency room of our hospital. Two of them were adolescents who attempted suicide through voluntary ingestion (one of them already in treatment with lithium and the other one, naïve). The third case was an accidental ingestion in an infant. Despite outcome was positive, all of them were hospitalized for close clinical and analytical monitorization, total intestinal lavage was performed in two, two required hyperhydration and one case had to undergo haemodialysis.

This paper reviews the particularities of acute lithium intoxication and its management in children and adolescents.
